The Infirmary

Stitches bought with gold, scars paid in silence.

The Infirmary is one of the essential town facilities in Darkest Dungeon, serving as the primary location where a hero's physical wounds can be stitched, set, and bandaged after a brutal delve into the dark. Unlike the delveable locations that populate the campaign map, the Infirmary is a fixed building within the town itself—a grim, candlelit ward where the game's battered adventurers return between expeditions to claw their way back toward combat readiness.

Its role in the campaign loop is straightforward but critical: every point of HP lost in the Catacombs, the Mine, the Fane, or any other dungeon must be purchased back here, gold by gold, before the next expedition. Without the Infirmary, even a perfectly managed party would bleed out across a handful of runs, making it the economic and logistical backbone of any long-term campaign.

Lore & Background

In the world of Darkest Dungeon, the darkness beyond the town walls does not merely threaten to kill; it mangles, poisons, and breaks. A hero who survives a delve into the Ruins or the Manor often returns with wounds that no field dressing can manage—deep lacerations, broken bones, infections that bloom black under the skin. The Infirmary exists to address exactly that. Its corridors are lined with iron gurneys, hanging lanterns that cast long amber shadows, and the faint copper smell of old blood that no amount of scrubbing can remove.

The healers who work its wards are not the robed, benevolent figures of fairy-tale hospitals. They are practical, exhausted, and paid by the hour. They set a dislocated shoulder with a grunt, cauterize a wound with a red-hot iron, and hand the bill to the hero before they have even finished groaning. There is no magic here, no divine intervention—just skilled hands, rough cloth, and the cold arithmetic of gold against flesh.

Over the course of a long campaign, the Infirmary becomes a character unto itself. The same hero who limped in with a shattered arm in the early Catacombs returns, months of delving later, to have a fresh gash above the eyebrow stitched shut while another hero is being bled and bled again in the next bed. The walls bear the memory of every wound that has passed through them, and the gold that funded each healing is a quiet ledger of how far the town has pushed its people into the dark.

In Their Own Story

The door groaned open on rusted hinges, and the smell hit first—iodine, old sweat, the sweet rot of a dressing left too long. The Vagabond dragged himself in, his left hand a useless smear of dried blood, and collapsed onto the nearest gurney. The healer did not look up from the ledger she was balancing. "Gold or the door," she said, and set a basin of water and a length of clean linen on the tray beside his hip. The Vagabond counted his coins. Fourteen. Enough for the arm, maybe not the ribs. The healer was already unwrapping a fresh roll of bandage, her jaw tight, her eyes the flat grey of a winter sky. Outside, the town bells rang the hour, and somewhere in the dark beyond the walls, something that had torn his side open was still walking, still hunting, still waiting for the next fool to come home bleeding. The Vagabond bit down on a leather strap and let the needle in.

Reader's Guide

The Infirmary is not a delveable location; it has no room grid, no enemy encounters, and no environmental hazards in the way the Catacombs or the Fane do. It is a fixed town building, and its "layout" is purely functional: you select a hero, choose how many HP to restore, and pay the gold cost. The interaction is immediate and transactional.

Its role in the campaign loop is nonetheless central. After every delve, the party returns with a mix of HP loss and stress gain. The Infirmary handles the HP; the Sanctuary handles the stress. Managing the gold spent at the Infirmary versus the gold saved for weapons, armor, and provisions at the Blacksmith or the Market is one of the core economic decisions in Darkest Dungeon. A mismanaged healing budget can leave a party too weak for the next delve, while over-spending on healing can starve the upgrades that keep them alive.

Geographically, the Infirmary sits in the town alongside the Tavern, the Sanctuary, the Blacksmith, and the other facilities. It is available from the very first day of a campaign and does not unlock at a later stage. Difficulty-wise, it does not scale with the dungeon tier; its cost per HP point remains a flat gold expense, which means its relative weight in your economy shifts as your income from deeper, more lucrative delves increases. In early-game runs, a full heal can consume a painful chunk of your gold; by the time you are farming the Fortress, the same heal is a rounding error. That shift is part of the campaign's natural economic curve.
